First Visit
Your first visit will consist of a consultation explaining your diagnosis and treatment options. Occasionally, surgery can be performed the same day as the consultation. However, a complex medical history or treatment plan may require an evaluation and a second appointment to provide treatment on another day.
First Consultation
When you arrive for your first visit, you will be greeted by our friendly office staff who will ensure that the proper registration information is collected for your consultation. Please assist us by providing the following information at the time of your consultation:
- Your referral slip from your dentist and any X-rays, if applicable
- A list of medications you are presently taking
- If you have insurance, please bring both your medical and dental cards with you.
- To protect against identity theft, we ask that you bring a driver’s license or other government-issued photo ID.
Fill out the patient paperwork and submit it to our office through the links below before your consultation.
During your consultation appointment, Dr. Brown, Dr. Porter, Dr. Fuentes, or Dr. Erickson will perform a dental examination, explain your diagnosis, and outline his recommended treatment options. Your doctor will help you determine the treatment plan that you feel comfortable with and choose the best option for anesthesia or sedation during the procedure.
It is important that a parent or guardian must accompany you to the consultation visit if you are under 18 years of age.
X-Rays
We will take either an X-Ray or CT scan with our state of the art imaging CBCT equipment or Panoramic equipment. These are taken at no extra cost to you and ensure that our surgeons have the best imaging with which to diagnose and treat you.
