Initial Consultation

What happens during my first consultation?

When you arrive for your first visit, you will be greeted by our friendly office staff who will ensure that the proper registration information is collected for your consultation. Please assist us by providing the following information at the time of your consultation:

  • Your referral slip from your dentist and any X-rays, if applicable
  • A list of medications you are presently taking
  • If you have insurance, please bring both your medical and dental cards with you.
  • To protect against identity theft, we ask that you bring a driver’s license or other government-issued photo ID.

To make the registration process run as smoothly as possible, you can download the patient registration form from our website and complete the form before your consultation.

During your consultation appointment, Dr. Brown, Dr. Porter, Dr. Fuentes, or Dr. Erickson will perform a dental examination, explain your diagnosis, and outline his recommended treatment options. Your doctor will help you determine the treatment plan that you feel comfortable with and choose the best option for anesthesia or sedation during the procedure.

It is important that a parent or guardian must accompany you to the consultation visit if you are under 18 years of age.
